Case management in psychiatry is a complex and person-centered service that aims to support people with mental disorders in their treatment journey by assigning responsibility for coordinating the treatment process to a single specialist – the case manager.

Through case management:
- ensuring smooth access for patients to necessary health care, social, and support services;
- avoidance of treatment discontinuation and support for continuity;
- An individual treatment plan is created based on the person's actual needs and coping abilities.
- The patient's family members and other network members are involved in the treatment process, if possible and necessary.
The case manager is a trained healthcare or social services professional who works with the patient, the treatment team, and external service providers.
Who is this service intended for?
We offer case management:
- people who are seeking psychiatric help for the first time;
- those whose treatment has been taken over from a family doctor or other system (e.g., unemployment insurance fund, social welfare);
- patients whose condition or needs have changed and whose treatment plan needs to be updated.
What does case management involve?
- initial contact with the patient and, if necessary, with their relatives;
- assessment of condition, coping, and resources;
- preparation and implementation of treatment plans;
- cooperation with various service providers;
- monitoring, counseling, and supporting treatment compliance;
- planning new interventions according to changes in the patient's condition.
The service is provided through both face-to-face meetings and remote consultations, taking into account the individual's needs and possibilities. The service is part of the work of the medical team and its purpose is to ensure that the patient receives timely and appropriate assistance, to prevent problems from worsening, and to support recovery..
